Required information for FDA registration of pet food in the U.S.

The following information is required when applying for FDA approval:

  1. Business License
  2. Fill out the FDA registration application form
  3. Dun & Bradstreet Code Request Form (Dun & Bradstreet code required to use FDA)
  4. Seal of the Dun & Bradstreet Authorization Letter

FDA Qualification Upload Compliance Guidelines

Note on the physical picture:

  • Take clear photos of the product in a real environment.
  • Labeling information and the actual picture should appear in the same photo and be clearly visible.
  • Photograph clear product labeling information, including but not limited to: product name, manufacturer's name, and manufacturer's address.
  • The information on the bag and the information on the adhesive label must be identical.

Note on qualification certificates:

  • Qualifications are proof of FDA registration and must be included:
  • Registration Number (11-digit Number)
  • Manufacturer's name and address
  • Within the validity period of certificate registration
  • Certificate must be signed/stamped
  • The product information in the physical drawing should be consistent with the information in the datasheet, e.g., manufacturer's name and address.
